@antp - with regards to the country of origin, deciding which country is "more important" will be too difficult in most cases. Perhaps you could create a new series of "dual flags" for common combinations, such as UK/USA, USA/CDN, etc. The flags would need to be similar in style and size to the current ones, but with a diagonal line splitting the flag in two. It could look something like this:
